The repair of groin hernias nowadays usually entails the reinforcement of the abdominal wall with a synthetic mesh in order to reduce the rate of recurrences over the years postoperatively. The laparoscopic operation by the so-called TAPP-approach offers a particularly minimal invasive procedure and femoral hernias can be fixed with the same approach. Alternatively, the mesh can be introduced by a small groin incision during the so-called Lichtenstein procedure. A reconstruction of the abdominal wall in the groin area e.g. by the Shouldice technique without the use of a mesh can be discussed in selected case. In either case, we give you all the information you need to make a decision for the technique you prefer. Particularly symptomatic umbilical hernias should be repaired. Small hernias with a diameter of less than 1.5cm can usually be close by a suture alone, while bigger hernias might require reinforcement with a synthetic mesh to reduce the recurrence rate. Most of these procedures can be carried out as an outpatient procedure. Hernias at the incision site after previous abdominal surgery are very common. In most cases the hernias are too big to be closed by a suture alone. Therefore, reinforcement with a synthetic mesh is necessary in most cases. Depending on patient related factors, the prevision operation, size and location of the hernia, the mesh can either be introduced by an open or minimal invasive, laparoscopic procedure. In some cases, the minimal-invasive, open MILOS-technique can be applied.
